Driving Evidence-Informed Practice for more than a dozen of Alberta’s Primary Care Networks

We are primary care evaluation experts.

 

We began working with Primary Care Networks (PCNs) in 2014, and haven’t stopped. We’ve supported over a dozen Alberta PCNs to understand their programs’ outcomes, improve processes, and report on all required “Schedule B” measures.

Our clients are large and small PCNs across all zones. We have recurring annual contracts with some and provide episodic, project-based services for others. Over the years we have conducted evaluations for our PCN clients related to:

  • Mental health, chronic disease management, maternity, pharmacy, nutrition, fitness and social work programs

  • Cross-disciplinary team effectiveness

  • Medical home implementation progress

  • Specialist linkage and referral processes

  • Social networks and partner relationships

  • Achievement of strategic goals
